Letter Form 100-3  To Credit Issuers: Requesting a Fraudulent Inquiry to be Removed

Please Send this letter form to the Credit Issuers or Merchants who appear on your credit report. Do not send it to the Identity Theft Resource Center as we cannot clear your record for you of fraudulent information.

As well, please see Letter Form 124D to request an inquiry be removed from your credit report by the three Credit Reporting Agencies.

Include with your letter a copy of your police report as well as a fraud affidavit. www.ftc.gov/bcp/conline/pubs/credit/affidavit.pdf . Mail all documents Certified Mail Return Receipt.
