Below is a sample letter to request a child’s credit report from the three credit reporting agencies. Please read Fact Sheet 120A to know what to include with this letter.

Always send items Certified Return Receipt Mail.

Please DO NOT send the ITRC any personal documents or letter forms as we cannot gather information for you or clear fraudulent activity that may appear.
